By signing this form, you acknowledge and agree to the following terms and conditions for grooming services provided by Fairy Tails Mobile Dog Grooming:

1. Consent for Grooming Services

I consent to have my pet groomed by Fairy Tails Mobile Dog Grooming. I understand that grooming involves the use of sharp tools, electrical equipment, and physical handling, which may pose a risk of injury despite all reasonable care being taken.

2. Potential Risks

While every effort is made to ensure your pet’s safety and comfort, I acknowledge that grooming may result in rare but possible outcomes such as:

Minor nicks, cuts, or clipper irritation
Stress or anxiety, especially in nervous, senior, or medically compromised pets
Accidental injury during grooming or handling

3. Matting and Shaving

I understand that if my pet has severely matted fur, it may need to be shaved to prevent pain or injury. I acknowledge that shaving may reveal pre-existing skin conditions such as irritation, rashes, or sores. I accept that Fairy Tails Mobile Dog Grooming is not liable for post-shaving effects or injuries caused by matting.

4. Fleas and Parasites

If fleas or other parasites are found on my pet during grooming, a flea treatment will be applied, and an additional fee will be charged. This is necessary to protect the grooming van, tools, and other clients' pets.

5. Liability Waiver

I release Fairy Tails Mobile Dog Grooming, its owner, and staff from any liability for:

Loss or injury to my pet during grooming
Pre-existing health conditions revealed during or after grooming
Any damages to personal property if services are performed at my home

6. Cancellations and No-Shows

Appointments must be cancelled or rescheduled at least 24 hours in advance. Cancellations with less than 24 hours’ notice may incur a $50 cancellation fee. No-shows or frequent cancellations may result in prepayment being required for future appointments.

7. Refusal of Service

Fairy Tails Mobile Dog Grooming reserves the right to refuse service at any time. This includes, but is not limited to, situations involving aggressive or extremely anxious pets, or when it is deemed unsafe or unethical to continue grooming. In such cases, the owner will be contacted and must arrange immediate collection of their pet.
